How do I obtain a copy of my child's immunization record?
Call me at 617.635.1650 x105 or send me a note with your child's name and date of birth.  Please allow a 24- hour turn around.

How can I contact a school nurse in any BPS?  Call the main BPS number 617.635.9000 and ask for the school your child attends.  Once you reach the school’s main office ask for the school nurse.

Why do I need to sign a new medication authorization form every school year?

 Administering medication in compliance with our Massachusetts Nurse Practice Act, states we do not administer medication (prescription or over the counter medications) and or medical treatment without a doctor's order.  In order to legally comply with school regulations, I need a parent/guardian signature for medication/treatment administration.  A new medication form must be completed and on file in the nurse's office for each school year prior to the administration of any medication/treatment. 


How does my child at the FREDERICK get his / her medication if he or she is going on a field trip?                                                                  

 Most students at the FREDERICK can be taught to self-administer their own medications.  I will assist students to attain the goal of self-administration of their medication.  For field trips or after-school activities, the parent-designated school staff should carry the student's medication so the student can take his / her own medication at the appropriate time.  Parent or guardian, school nurse and primary care provider must sign off on this procedure.


Why does my child need to provide the school nurse with a physical every year?
 
All students must provide the school nurse with a current physical because the needs and health of your child may have changed from last year.  Physicals are acceptable for ONE CALENDAR YEAR. 


Why does my child need to provide the school nurse with a sport’s physical every year?  He or she played this sport last year.

 All students in grades 6-8 who plan on playing after school intramural or interscholastic sports must have a physical examination by their primary care provider (MD or NP).   Physicals are acceptable for ONE CALENDAR YEAR.  Health questionnaires are available in the nurse's office. We need to know if there have been any changes in the health of your child or new information on your child.
